胺碘酮联合稳心颗粒治疗老年冠心病合并心律失常的效果观察

摘要目的观察胺碘酮、稳心颗粒用于冠心病、心律失常老年患者的治疗效果。方法选取我院门诊部2014年6月～2016年7月收治的85例老年冠心病、心律失常患者为研究对象，按随机排列表法分为对照组和观察组。对照组42例（使用胺碘酮治疗），观察组43例（以稳心颗粒+胺碘酮治疗），观察治疗前后患者心绞痛、心律失常（室性早搏、短阵室性心动过速）改善情况，检测两组血流动力学指标水平变化，比较治疗期间不良事件发生率。结果入院时两组心绞痛、室性早搏、短阵室性心动过速差异无统计学意义（P＞0.05），治疗后观察组心律失常、心绞痛治疗效果优于对照组，差异有统计学意义（P＜0.05）；入院时两组血液流动力学指标血浆比粘度、低切黏度、高切黏度、纤维蛋白原、血细胞比容显差异无统计学意义（P＞0.05），治疗后观察组血液流动力学指标及改善效果均优于对照组，差异有统计学意义（P＜0.05）；不良事件观察组发生率为9.3%，对照组为14.3%，差异无统计学意义（χ2=1.201，P＞0.05）。结论老年冠心病、心律失常患者以胺碘酮、稳心颗粒联合治疗心律失常得到改善，血液流动力学恢复，不良反应少。

Abstract：Objective To observe the effect of Amiodarone and Wenxin Granule in the treatment of coronary heart disease and arrhythmia in elderly patients.Methods 85 cases of elderly patients with coronary heart disease and arrhythmia who were admitted to the outpatient in our hospital from June 2014 to July 2016 were selected as the study subjects.According to the random arrangement table method,the patients were divided into control group and observation group.42 patients in the control group were given Amiodarone for treatment,and 43 patients in the observation group was given Wenxin Granules+Amiodarone for treatment.Before and after treatment,the improvement of angina and arrhythmia(ventricular number，short array of ventricular speed)was observed before and after the treatment.The changes of hemodynamic indexes were measured in both groups，and the records of adverse events during treatment was compared.Results At the time of admission，the number of angina pectoris,times,the number of premature ventricular contraction,and the number of short array of ventricular fibrillation,there were no significant difference compared with the control group (P＞0.05).After treatment，the indexes in the observation group respectively，the curative effect of arrhythmia and angina pectoris was better than that in the control group,with significant difference (P＜0.05);upon admission，the hemodynamic index of plasma specific viscosity in the observation group,low shear viscosity,high shear viscosity fibrinogen,hematocrit were no significant differences compared with those in the control group,with significant difference (P＞0.05).After treatment，the improvement of hemodynamic indexes in the observation group were all better than those in the control group,with significant difference (P＜0.05);the incidence rate in the observation group was 9.3%,compared with 14.3%in the control group,there was no significant difference between the two groups (χ2=1.201,P＞0.05).Conclusion After the combined treatment of amiodarone and wenxin granule for elderly patients with coronary heart disease and arrhythmia，the arrhythmia is improved,hemodynamics is recovered,and the adverse reactions are less.
