慢性心力衰竭患者红细胞分布宽度、血小板平均体积水平变化及其与血浆N端前脑钠肽相关性分析

摘要目的分析慢性心力衰竭血小板平均体积（MPV）与红细胞分布宽度（RDW）水平变化特征，探讨其与血浆N端前脑钠肽（NT-proBNP）的相关性。方法选取2018年2月—2020年12月于芜湖市繁昌区人民医院内科就诊的78 例慢性心力衰竭患者作为研究对象，按照美国纽约心脏病学会（NYHA）心功能分级将患者分为Ⅰ/Ⅱ级（43例）与Ⅲ/Ⅳ级（35 例），于同期选取78 例健康体检者作为健康对照组。收集患者入院后的MPV、RDW 与血浆NT-proBNP 检测结果，比较三组患者的MPV、RDW 与血浆NT-proBNP 水平，分析其相关性。结果慢性心力衰竭患者RDW、MPV 水平及血浆NT-proBNP 均高于健康对照组，差异有统计学意义（P<0.05）；Ⅲ/Ⅳ级患者RDW、MPV 水平及血浆NT-proBNP 均高于Ⅰ/Ⅱ级，差异有统计学意义（P<0.05）。慢性心力衰竭患者的MPV 水平与血浆NT-proBNP 成正相关（r=0.223，P=0.048）；RDW 水平与血浆NT-proBNP 成正相关（r=0.544，P=0.001）。结论 RDW、MPV 对慢性心力衰竭患者的早期诊断与病情评估具备重要价值。

Abstract：Objective To analyze the changes of mean platelet volume(MPV)and red blood cell distribution width(RDW)levels in chronic heart failure,and to explore its relationship with plasma N-terminal pro-brain natriuretic peptide(NT-proBNP).Methods A total of 78 patients with chronic heart failure who were treated in the Department of Internal Medicine of Fanchang District People′s Hospital of Wuhu City from February 2018 to December 2020 were selected as the research objects.According to the New York Heart Association(NYHA)cardiac function classification,the patients were divided into grade Ⅰ/Ⅱ(43 cases)and grade Ⅲ/Ⅳ(35 cases).During the same period,78 healthy subjects were selected as the healthy control group.The MPV,RDW and plasma NT-proBNP detection results of patients after admission were collected,and the MPV,RDW and plasma NT-proBNP levels of the three groups of patients were compared,and their correlation was analyzed.Results RDW,MPV levels and plasma NT-proBNP in chronic heart failure patients were higher than those in healthy control group,and the differences were statistically significant(P<0.05).The levels of RDW,MPV and plasma NT-proBNP in grade Ⅲ/Ⅳwere higher than those in grade Ⅰ/Ⅱ,and the differences were statistically significant(P<0.05).The level of MPV in chronic heart failure patients was positively correlated with plasma NT-proBNP(r=0.223, P=0.048);RDW level was positively correlated with plasma NT-proBNP(r=0.544, P=0.001).Conclusion RDW and MPV have important value in the early diagnosis and evaluation of chronic heart failure.
