孕妇中医体质与优生优育的关系

摘要目的调查广州市荔湾区华林街常住孕妇中医体质与优生优育的关系。方法使用《中医体质辨识量表》对2010年10月～2015年9月于广州市荔湾区华林街社区卫生服务中心建立《广州市孕产妇保健系统管理手册》的孕妇进行调查，依据《中医体质分类判定标准》进行中医体质辨识，并追踪其孕产期情况及新生儿情况，分析孕期平和体质与偏颇体质对优生优育的影响。结果本次调查有效对象共1892例，其中平和质573例，占30.29%，偏颇质1319例，占69.71%，偏颇体质前三位分别为阳虚质、气虚质、湿热质；高危妊娠、孕产期并发症及合并症、不良妊娠结局、早产、低出生体重、Apgar评分≤7分的平和质分别为142、80、11、24、25、8例，发生率分别为24.78%、13.96%、1.92%、4.19%、4.36%、1.40%，偏颇体质分别为437、249、57、94、91、61例，发生率分别为33.13%、18.88%、4.32%、7.13%、6.90%、4.62%，两者差异均有统计学意义（P＜0.05）。结论平和体质更利于优生优育。

Abstract：ObjectiveTo evaluate the relationship between the constitution of the pregnant woman and better born and better upbringing in Liwan District Hualin street of Guangzhou.MethodsFrom October 2010 to September 2015,the TCM Constitution Identification Scale was used to investigate the constitution of the pregnant woman when they first come to build up pregnant file and gave constitution identification of traditional Chinese medicine according to TCM constitution classification and decision standard in the Community Health Center of Liwang District Hualin Street,then the situation in prenatal,postnatal and the situation of newborn was followed up,the influence of the constitution of yinyang harmony and yin-yang unharmony in the pregnant woman to prenatal and postnatal was evaluated.ResultsIn total 1892 effective cases,573 cases(30.29%)were in the constitution of yin-yang harmony and 1319 cases(69.71%)were in the constitution of yin-yang unharmony.The top three of the constitution of yin-yan unharmony were constitution of yang asthenia,constitution of qi asthenia,constitution of damp-heat.In the constitution of yin-yang harmony pregnant woman there were 142 cases with high risk pregnant,80 cases occured coplication prenatal and postnatal,11 cases of adverse pregnant outcome,24 cases of premature birth,25 cases of low-birth-weight,8 cases of Apgar scores≤7 points. The corresponding incidence rate were 24.78%,13.96%,1.92%,4.19%,4.36%,1.40%.While in the constitution of yinyang unharmony pregnant woman there were 437 cases with high risk pregnant,the other corresponding figures were 249,57,94,91,61.The corresponding incidence rate were 33.13%,18.88%,4.32%,7.13%,6.90%,4.62%,there were significant differences between yin-yang harmony and yin-yang un-harmony (P＜0.05).ConclusionThe constitution of yinyang harmony is better for good birth and good care.
