植入式静脉输液港与经外周静脉中心静脉置管在妇科恶性肿瘤化疗中的应用效果比较

摘要目的探讨植入式静脉输液港（VAP）与经外周静脉中心静脉置管（PICC）在妇科恶性肿瘤化疗中的应用效果。方法选取2018年10月～2019年10月江西省肿瘤医院住院化疗的100例妇科恶性肿瘤患者，按随机数字表法分为PICC 和VAP组，每组各50例。PICC组接受PICC 化疗及护理，VAP组采取VAP 化疗及护理，观察6个月。比较两组的置管操作、留置情况、维护操作时间及并发症发生情况。结果 VAP组的置管维护时间为（11.25±2.14）min，短于PICC组的（14.79±3.08）min，差异有统计学意义（P＜0.05）；VAP组的一次性置管成功率（98.00%）与PICC组（94.00%）比较，差异无统计学意义（P＞0.05）；VAP组6个月内留置率（86.00%）高于PICC组（68.00%），差异有统计学意义（P＜0.05）；VAP组的并发症总发生率（8.00%）低于PICC组（24.00%），差异有统计学意义（P＜0.05）。结论 VAP 在妇科恶性肿瘤化疗中的应用效果较PICC 好，可提高留置率，缩短置管维护时间，降低并发症的发生。

Abstract： Objective To explore of application effect of implantable intravenous infusion port (VAP) and peripherally inserted central catheter (PICC) in chemotherapy of gynecological malignant tumors.Methods From October 2018 to October 2019，100 patients with gynecological malignant tumors who were admitted to Jiangxi Provincial Cancer Hospital for chemotherapy were selected.They were divided into PICC and VAP groups according to the random number table method，50 cases in each group.In the PICC group，patients received PICC chemotherapy and nursing，and in the VAP group，participants received VAP chemotherapy and nursing.The observation lasted 6 months.The operation，retention，maintenance operation time and complications of the two groups were compared.Results The operation time of cathetering maintenance in the VAP group was(11.25±2.14)min，shorter than that in the PICC group of(14.79±3.08)min，with statistical difference (P＜0.05).In the VAP group，the success rate of one-time cathetering was 98.00%，not statistically different from that in the PICC group accounting for 94.00% (P＞0.05).The retention rate of the VAP group within 6 months was 86.00%，higher than that in the PICC group accounting for 68.00%，and the difference was statistically significant (P＜0.05).The total incidence of complications in the VAP group was 8.00%，lower than that in the PICC group accounting for 24.00%，and the difference was statistically significant (P＜0.05).Conclusion The application effect of VAP in chemotherapy of gynecological malignant tumors is better than that of PICC，which can increase the retention rate，shorten the operation time of catheter maintenance and reduce the occurrence of complications.
