快速康复外科理念下围手术期护理干预在天玑骨科机器人辅助治疗胸腰椎骨折中的应用效果

摘要目的探讨快速康复外科理念下围手术期护理干预在天玑骨科机器人辅助治疗胸腰椎骨折中的应用效果。方法选取2020年11月至2021年11月就诊于莆田学院附属医院的60 例行天玑骨科机器人辅助治疗胸腰椎骨折的患者作为研究对象，采用随机数字表法将其分为对照组与观察组，每组各30 例。对照组采用常规护理干预，观察组采用快速康复外科理念下围手术期护理干预，比较两组患者的生理功能恢复情况（拔除尿管时间、首次进食时间、离床下地时间、出院时间）、术后第1、3 天视觉模拟评分法（VAS）评分、围手术期并发症发生情况及护理满意度。结果观察组术后拔除尿管时间、首次进食时间、离床下地时间、出院时间均短于对照组，差异有统计学意义（P＜0.05），观察组术后第1、3 天的VAS 评分均低于对照组，差异有统计学意义（P＜0.05）。观察组围手术期并发症总发生率低于对照组，差异有统计学意义（P＜0.05）。观察组围手术期护理满意程度高于对照组，差异有统计学意义（P＜0.05）。结论天玑骨科机器人辅助治疗胸腰椎骨折患者采取快速康复外科理念下围手术期护理干预，能够有效地缩短术后恢复时间，减轻疼痛，降低围手术期并发症。

Abstract：Objective To evaluate the effect of perioperative nursing intervention under the concept of enhanced recovery after surgery for thoracolumbar fractures treated with the TiRobot.Methods Sixty patients with Thoracolumbar fractures treated with the TiRobot from November 2020 to November 2021 in Affiliated Hospital of Putian University were selected,and they were divided into a control group and an observation group using the random number table method,with 30 cases in each group.In the control group,conventional nursing intervention was used,and in the observation group,perioperative nursing intervention under the concept of ERAS was used.Time of urinary catheter removal,time of first feeding,time of getting off the floor,and time of discharge was recorded and analyzed in both groups.The degree of pain was scored by visual analog scale (VAS) score on the 1st,3rd day after the operation.Complications and satisfaction with nursing care in both groups were observed and recorded.Results The time of urinary catheter removal,time of first feeding,time of getting off the floor,and time of discharge after surgery in the observation group were shorter than those in the control group,and the differences were statistically significant (P＜0.05).The VAS scores in the first day and third day after the operation in the observation group were lower than in the control group,and the differences were statistically significant (P＜0.05).The total incidence of perioperative complications in the observation group was lower than in the control group,and the difference was statistically significant (P＜0.05).The degree of satisfaction of perioperative nursing care in the observation group was higher than that in the control group,with statistically significant difference (P＜0.05).Conclusion The perioperative care intervention under the concept of ERAS in patients with thoracolumbar fractures treated with the TiRobot assisted can effectively shorten the postoperative recovery time,reduce pain and decrease perioperative complications.
