针刺联合中药熏洗坐浴在混合痔患者术后的应用

摘要目的探讨针刺联合中药熏洗坐浴在混合痔患者术后的应用效果。方法选择2017年1月～2019年6月在江西中医药大学第二附属医院行手术治疗的84例混合痔患者，按随机数字表法分为A组与B组，各42例。B组使用高锰酸钾溶液熏洗治疗，A组行针刺结合中药熏洗坐浴治疗。比较两组的症状改善时间、肛门坠胀持续时间、肛门坠胀程度、肛门疼痛、炎症反应及不良反应。结果 A组的水肿、疼痛、渗出物消失时间与创面愈合时间、肛门坠胀持续时间短于B组，肛门坠胀程度与肛门疼痛评分低于B组，差异有统计学意义（P＜0.05）；A组的白细胞介素-17（IL-17）、肿瘤坏死因子-α（TNF-α）水平低于B组，差异有统计学意义（P＜0.05）；治疗中，两组均未出现不良反应。结论混合痔术后患者行针刺结合中药熏洗坐浴治疗能有效缩短临床症状改善时间，减轻肛门坠胀程度与疼痛程度，降低炎症因子水平，利于患者病情恢复，具有较高的安全性。

Abstract：Objective To investigate the efficacy of acupuncture combined with traditional Chinese medicine fumigation bath in patients with mixed hemorrhoids after surgery.Methods A total of 84 patients with mixed hemorrhoids who underwent surgery in the Second Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from January 2017 to June 2019 were selected and divided into group A and group B according to the random number table method,with 42 patients in each group.Group B was treated with Potassium Permanganate Solution for fumigation and bathing,and group A was treated with acupuncture combined with Chinese medicine for fumigation and bathing.The symptoms improvement time, duration of anal swelling, degree of anal swelling, anal pain, inflammation and adverse reactions were compared between the two groups.Results The time of edema, pain, exudate disappearance, wound healing time and anal distension duration in group A were shorter than those in group B, the degree of anal swelling and anal pain score in group A were lower than those in group B, with significant differences (P＜0.05).The levels of interleukin-17(IL-17) and tumor necrosis factor-α (TNF-α) in group A were lower than those in group B, and the differences were statistically significant (P＜0.05).During treatment, there were no adverse reactions in both groups.Conclusion Acupuncture combined with traditional Chinese medicine fumigation and bathing treatment for patients with mixed hemorrhoids can effectively shorten the time to improve clinical symptoms, reduce the degree of anal swelling and pain,reduce the level of inflammatory factors, and help patients recover, which has high security.
