单纯后路与前后路联合手术治疗脊柱结核患者的效果与安全性比较

摘要目的比较单纯后路与前后路联合手术治疗脊柱结核患者的效果与安全性。方法选取2019年2月—2020年8月赣州市第五人民医院收治的70 例脊柱结核患者作为研究对象，按照随机数字表法分为对照组（35 例）与观察组（35 例），对照组采取单纯后路手术治疗，观察组采取前后路联合手术治疗。比较两组的手术指标、Cobb 角变化及并发症发生情况。结果观察组的手术时间、住院时间长于对照组，术中出血量多于对照组，差异有统计学意义（P＜0.05）；两组的并发症总发生率比较，差异无统计学意义（P＞0.05）；术后观察组的Cobb 角小于对照组，矫正度数大于对照组，矫正率高于对照组，差异有统计学意义（P＜0.05）。结论单纯后路治疗脊柱结核的手术时间及住院时间较短，术中出血量较少，而前后路联合治疗在清除病灶、矫正畸形方面的效果更佳。二者各有优势，且安全性较高，可根据患者情况选择适宜的手术方式。

Abstract：Objective To compare the efficacy and safety of only posterior approach and posterior combined anterior approach surgery in the treatment of patiens with spinal tuberculosis.Methods A total of 70 patients with spinal tuberculosis admitted to the Fifth People′s Hospital of Ganzhou City from February 2019 to August 2020 were selected as the research subjects.According to random number table method, they were divided into control group (35 cases)and observation group (35 cases).The control group was treated with posterior operation alone, and the observation group was treated with combined anterior and posterior operation.The surgical parameters,Cobb angle changes and complications were compared between the two groups.Results The operation time and hospitalization time of the observation group were longer than those of the control group, and the amount of intraoperative blood loss was more than that of the control group, the differences were statistically significant (P＜0.05).There was no significant difference in the total incidence of complications between the two groups (P＞0.05).Cobb angle of the observation group was lower than that of the control group, the degree of correction and the correction rate of the observation group were higher than those of the control group, the differences were statistically significant (P＜0.05).Conclusion Posterior treatment alone has shorter operation time and hospital stay, and less intraoperative blood loss, while combined anterior and posterior treatment is more effective in removing lesions and correcting deformities.Both of them have their own advantages and high safety,and the appropriate surgical methods can be selected according to the patient′s situation.
