太极拳运动对糖尿病辅助治疗作用的研究进展

[摘要]糖尿病是重要的慢性非传染病之一,其患病率和患者绝对数量逐年增加。随着病情进展,糖尿病可影响全身各个脏器组织,其治疗措施主要包括运动、饮食、教育、监测和药物治疗。太极拳作为一种运动疗法,是糖尿病健康运动管理中的主要措施之一,可作为干预或辅助治疗措施。本文对太极拳运动在糖尿病患者治疗中作用的研究进展作一综述。

[Abstract]Diabetes mellitus is one of the important chronic non-communicable diseases.The prevalence and number of patients increase year by year.As the disease progresses,diabetes can affect all organs and tissues of the body.Its treatment measures include exercise,diet,education,monitoring and medication.Tai Chi,as a kind of exercise therapy,is one of the main measures in the management of diabetes healthy exercise and can be used as intervention or adjuvant treatment.This paper reviews the recent progress in the role of Tai Chi exercise in diabetes therapy.

糖尿病是重要的慢性非传染病之一[1],其患病率逐年增加,给人类健康带来了严重的社会公共卫生问题。流行病学调查结果显示,我国20岁以上人群中糖尿病患病率为9.7%,成人糖尿病患者总数达9240万,数量居全球之首。糖尿病的治疗措施主要包括运动、饮食、教育、监测和药物治疗。
太极拳是一种传统的中国武术,其具有放松、深度调节呼吸技术等作用,属于低-中度强度的运动,可提高有氧运动能力[2],提高部分疾病患者的生活质量,如乳腺癌[3]、高血压[4]和骨质疏松[5]等,而且对帕金森病[6]、中风康复[7]、痴呆[8]等疾病的治疗及提高老年人认知能力方面[9]均具有良好改善作用,另外也有益于抑郁症的改善[10]和睡眠质量的提高[11]。太极拳作为一种运动疗法,是糖尿病健康运动管理中的主要措施之一,是一种低强度、低冲击性的运动项目,易于被患者接受[12],因此在糖尿病患者运动管理中,其可以作为干预或辅助治疗的措施。现对太极拳运动在糖尿病患者中作用的研究进展进行综述如下。
1 对糖尿病空腹血糖和胰岛素的作用
2型糖尿病是一种慢性炎症相关性的代谢综合征,可导致严重的并发症和致残[13],而太极拳作为运动疗法在2型糖尿病健康管理中具有调节血糖的作用。运动疗法可作为2型糖尿病患者最基础的非药物治疗措施,但太极拳作为运动疗法中一种技能性运动疗法,其在血糖调节方面的作用尚存在争议。
1.1 太极拳对糖尿病患者血糖监测指标具有良好的调节作用
研究显示,太极拳不但对糖尿病患者空腹血糖具有显著的改善作用[14],而且对伴有周围神经病变的2型糖尿病患者血糖也具有调节作用。多个研究证明经过太极拳运动干预糖尿病患者后,监测到糖化血红蛋白水平明显改善[15]。Liu等[16]在伴有血糖异常的代谢综合征患者中引入太极拳训练,监测的HbA1c和胰岛素抵抗情况均显示明显改善。
1.2 太极拳对糖尿病患者血糖检测指标无改善作用
Alsubiheen等[17]的研究发现太极拳干预后的糖化血红蛋白、胰岛素抵抗在统计学上没有显著性差异。Lee等[18]通过荟萃分析,提示并没有充足的证据证明太极拳可改善糖尿病患者的空腹血糖、降低患者的胰岛素抵抗,并且太极拳训练后血红蛋白也没有明显变化[19]。由于太极拳在控制血糖疗效方面的临床试验证据不足,还需要大量临床数据进行规范化研究,以确定太极拳在调节2型糖尿病患者血糖方面是否有益处。
太极拳作为2型糖尿病管理中的运动疗法,对血糖的控制具有一定的作用。但大多数临床试验中存在研究样本量较小、研究时间的限制、偏倚风险等因素,导致糖尿病患者的空腹血糖、糖化血红蛋白的研究结果存在严重的偏差,故需要进行大样本及严谨的随机临床试验,以进一步明确太极拳运动对糖尿病患者血糖的影响。
2 对糖尿病周围神经病变的影响
周围神经病变是糖尿病常见并发症之一,其症状发展缓慢和隐匿,不易被识别[1],通常表现为感觉障碍,严重时影响姿势稳定性和保护性反射。研究显示,太极拳可以改善足部感觉神经功能及小腿反射[20]、缓解糖尿病患者本体感觉受损情况[21],糖尿病患者通过太极拳运动疗法可获得更多益处。
2.1 太极拳改善糖尿病患者周围神经传导速度
Hung等[14]发现经过太极拳干预,糖尿病患者的神经传导速度明显增加,特别是双侧胫神经和双侧尺神经的运动神经和远端感觉神经的传导速度。当太极拳联合心理意象(以往感觉/知觉上的经验在脑中的重现或回忆),Hoffmann反射中的神经传导点位振幅显著增加;腓肠神经干预前、后的速度、振幅、潜伏期明显改善[22]
2.2 太极拳改善糖尿病患者周围神经感觉及运动
通过太极拳对2型糖尿病的干预,发现踝关节本体感觉及足底感觉显著改善[23],振动阈值显著降低,太极拳训练对糖尿病患者的外周感觉有明显的改善作用。
对39例2型糖尿病周围神经病变患者进行12周太极拳训练,发现单腿站立平衡时间延长,糖尿病患者躯体平衡改善,并且延长单侧肢体姿势维持时间,随着时间的持续,躯体平衡改善更为明显[24]。最大步态速度随着时间的推移同样也出现显著的改善,特别是生活质量差的糖尿病患者在平衡感觉方面更具有显著改善。在2型糖尿病伴远端对称性多发性神经病变患者,太极拳显示8英尺起立行走持续时间减少(即行走速度更快),单腿支撑时间增加[24]。通过荟萃分析显示,太极拳可促进糖尿病周围神经病变患者的步态、平衡和躯体功能活动的改善,对糖尿病周围神经病变患者在预防跌倒中起重要作用。当太极拳联合心理意象干预,显示出活动平衡信息量表、单腿直立试验、功能性伸展试验均有明显改善(P<0.01)[22-25],在平台平衡测试中,不同训练任务下的糖尿病患者的平衡感觉具有不同程度的改善[25]
综上所述,在糖尿病运动管理中,太极拳运动疗法对糖尿病患者的周围神经传导速度、周围感觉、平衡觉等具有明显改善作用,且有促进健康的潜力,因此可通过规范管理太极拳运动疗法,以指导糖尿病患者健康运动。
3 对糖尿病患者生活质量和社会功能的影响
实现和维持正常的日常生活是2型糖尿病管理重要目标之一,较高水准的躯体活动可以改善糖尿病患者的生活质量。太极拳作为有氧运动对糖尿病患者的生活质量具有一定的改善作用[26]
Liu等[27]对糖尿病患者进行太极拳训练,发现社会功能量表中的躯体功能、躯体角色、躯体疼痛和躯体活力明显改善 (P<0.05),躯体活力的平均差异为385.94分/周,躯体角色功能达到18.6;干预前后的社会功能量表平均差异为5.5(P<0.05)。同时显示自我护理活动显著提高(P<0.05),社会功能量表中的心理成分总结、社会功能、情绪角色限制、精神健康和活力均具有明显的改善。由此可见,太极拳运动在糖尿病运动管理中对部分社会功能,如躯体功能、躯体角色、躯体疼痛和躯体活力等方面具有明显的改善,同时为糖尿病患者提供自我保健活动,提高其生活质量。
4 与糖尿病患者精神症状的关系
精神心理问题与2型糖尿病患者的血糖控制有密切的影响。有研究显示,2型糖尿病患者患抑郁症的风险较非糖尿病患者增加24%;然而较高水平的躯体活动可降低糖尿病患者的抑郁症状[28]。太极拳作为运动疗法在糖尿病管理中不仅可以改善临床症状,而且还可以改善精神心理症状。Liu等[16]在代谢综合征伴有血糖异常的患者中进行太极拳训练,发现健康相关生活质量评价中的躯体健康、精神健康、活力及精神总分均显著改善(P<0.05);应激和抑郁症状量表的测量也显示具有显著差异性,提示太极拳可改善抑郁症患者的情绪,使量表的测量值从60%下降到20%。因此,太极拳对伴有精神症状的糖尿病患者,特别是抑郁症状具有一定的改善作用。
5 对糖尿病患者免疫系统的影响
2型糖尿病患者常常伴随着免疫系统的变化,特别是T细胞免疫。Ho等[29]通过在正常的成年志愿者中开展太极拳训练,发现调节性T细胞功能增强。Hojman[30]报道,对于久坐的老年人进行为期6个月的监督运动训练可促使免疫系统功能的提升,并且未发现存在不利影响,反而有益于自然杀伤细胞系统的功能。太极拳作为一种低强度运动项目,可增强T细胞免疫功能并减少呼吸道感染。对于糖尿病患者选择最佳运动可诱导某些抗炎介质生成,提高细胞免疫力。
6 小结
综上所述,太极拳作为糖尿病患者运动管理的一种方法,具有重要作用,对于神经系统的感觉、平衡及免疫系统等方面均具有明显的改善效果。但由于大多数研究样本量较小,研究时间限制等各种因素影响,显示太极拳对糖尿病患者的空腹血糖、糖化血红蛋白等无显著性的影响。故还需要结合患者规范化管理、长时间锻炼及其他相关参数进行综合全面评估,以确定太极拳是否对糖尿病患者存在潜在性的影响。
